Wells Fargo Health Benefit Services is the custodian of our current Health Savings Accounts (HSA's).  Their website has lots of information about HSA's, links, forms, and Frequently Asked Questions.  Forms most often requested include  Contribution/Rollover Form and Reimbursement Request Form.

If you want to have online access to your Wells Fargo HSA to view transactions, balances, etc. go to Wells Fargo Health Account Manager.  Go here to register.

For assistance with your cafeteria plan, contact the administrator via their website where you can check your cafeteria plan balance on line.  Forms most requested related to the cafeteria plan are the Enrollment Form and the Claim Form.  Download instructions on registering with www.myrsc.com.  They also may be contacted at:

United Health Care is the provider of our health insurance.  Their website is quite useful in locating facilities, examining benefits provided by our policy, accessing claim history, etc.  Click on the 'Register' link on the left side of the home page and then login to view your personal account information, view claims, find a doctor and even refill a prescription.  Click the link to download a Benefit Summary of the 2009 or 2008 plan.

Wells Fargo Insurance Services is the third party administrator of our group health insurance plan.  They can field any question you may have regarding our health insurance policy including, covered services and products, prescriptions and billing disputes.  They may be contacted at:
